At the heart of Private Care Management is the commitment to provide seniors with the comfort and familiarity of their own homes while reducing the stress on families and caregivers. Just as with our standard care management approach, an initial assessment takes place within the candidate’s home. We work collaboratively with families and caregivers to ensure every aspect of care aligns with the individual’s preferences and requirements.

  • At-Home Initial Assessment: Our Certified Private Care Manager initiates the process with a personalized at-home assessment. This assessment provides you and your family with expert insight into the current environment and offers solutions to tailor recommendations for managing day-to-day living while also planning for services that may be required in the future.

  • Safety and Security: Keeping you or your loved ones safe at home is our top priority. Our experienced team is experienced in preventative measures for maximum peace of mind by implementing solutions to increase security and safety.

  • Advocating for Health and Well-Being: The dedicated Private Care Manager will assist with navigating the complexities of the healthcare system to ensure you or your loved one’s voice, concerns, and needs are heard and addressed. We work with medical professionals to facilitate doctor appointments, assess appropriate needs for treatments, monitor prescriptions and instructions, and maintain ongoing communication with the family.

  • Financial Assistance Management: We alleviate the stress of managing finances by offering consultations with an accountant to assess the financial structures required to cover medical expenses and associated fees.

Jewish Family and Children’s Services of Northern New Jersey provides Kosher Meals on Wheels, ADRC Care Management and Counseling services through the Bergen County, Division of Senior Services at no charge to eligible clients.

JFCS is a licensed mental health agency. Most insurances accepted – Medicaid, Medicare and private insurance. For those without insurance coverage, JFCS has a sliding fee scale.
